Has anyone encountered a malicious or hacked smart contract?

I recently learned that it’s possible to create malicious smart contracts. I also understand that it's possible to hack smart contracts and exploit them for malicious intent. Curious if anyone has encountered either of these situations… What happened? Did you lose ETH or other assets?

I'm looking into building something to combat this.
