On Vitalik’s definition of Ethereum

Vitalik has defined Ethereum as a general purpose blochchain. My understanding is that a blockchain is basically a distributed database. Based on this my question is: what does it mean that apps are built on top of Ethereum if it is a database? How can apps run on top of Ethereum if it is a database?
